Abstract

ÀÌ ³í¹®ÀÇ ¸ñÀûÀº ¼ºÀå±â °ñ°Ý¼º III±Þ ºÎÁ¤±³ÇÕÀÇ Ä¡·á¸¦ À§ÇÏ¿© »õ·ÎÀÌ °í¾ÈµÈ Tandem Traction Bow Appliance(TTBA)ÀÇ »ó¾Ç°ñ °ßÀÎ È¿°ú¸¦ Æò°¡ÇÏ´Â °ÍÀÌ´Ù. ÀÌÈ­¿©ÀÚ´ëÇб³ ¸ñµ¿º´¿ø Ä¡°ú ±³Á¤°ú¿¡ ³»¿øÇÑ ¼ºÀå±â °ñ°Ý¼º III±Þ ºÎÁ¤±³ÇÕ È¯ÀÚ Áß TTBA·Î Ä¡·á¹ÞÀº 88¸í(¼Ò³â 42¸í, ¼Ò³à 46¸í)À» ´ë»óÀ¸·Î ÇÏ¿´´Ù. Ä¡·á ½ÃÀÛ ½Ã ¿¬·ÉÀº 7¼¼ 6°³¿ù{pm}1³â 6°³¿ùÀ̾úÀ¸¸ç, Æò±Õ Ä¡·á ±â°£Àº 13°³¿ù{pm}3°³¿ùÀ̾ú´Ù. Ä¡·á ÀüÈÄÀÇ º¯È­¸¦ ¾Ë¾Æº¸±â À§ÇÏ¿© paired sample t-test¸¦ ½Ç½ÃÇÏ¿´À¸¸ç ´ÙÀ½°ú °°Àº °á·ÐÀ» ¾ò¾ú´Ù. »ó¾Ç°ú »ó¾ÇÄ¡¿­Àº Àü¹æÀ¸·Î À̵¿ÇÏ¿´´Ù. ÇϾÇÀº ÈĹæÀ¸·Î À̵¿ÇÏ¿´À¸³ª, ÀÌ ¾çÀº Åë°èÇÐÀûÀ¸·Î À¯ÀÇÇÏÁö ¾Ê¾Ò´Ù. ÇϾÇÄ¡¿­Àº Àü¹æÀ¸·Î À̵¿ÇÏ¿´´Ù. Net dental changes¿Í apical vase change°¡ º¹ÇÕµÇ¾î ³ªÅ¸³­ °á°ú, III±Þ ºÎÁ¤±³ÇÕ Ä¡·á¿¡ À¯¸®ÇÑ ¹æÇâÀ¸·Î total molar relationship correctionÀÌ ÀϾ´Ù. ÀÌ»óÀÇ °á°ú´Â TTBA Ä¡·á°¡ »ó¾Ç°ñ °áÇÌÀ» °¡Áø ¼ºÀå±â °ñ°Ý¼º III±Þ ºÎÁ¤±³ÇÕ¿¡¼­ À¯ÀÇÇÑ »ó¾Ç°ñ °ßÀÎ È¿°ú¸¦ °¡Áø´Ù´Â °ÍÀ» ÀǹÌÇÑ´Ù.

Objective: The purpose of this study was to investigate the maxillary protraction effects of the Tandem Traction Bow Appliance (TTBA), a new appliance devised several years ago for the treatment of growing skeletal Class III patients.
Methods: Participants were 88 Korean children (42 boys, 46 girls) with skeletal Class III malocclusion treated with TTBA at the orthodontic clinic of Ewha Womans University Mokdong Hospital. Mean age at the start of treatment was 7.5 years{pm}1.5 years. Mean treatment periods were 13{pm}3 months. Pretreatment and posttreatment lateral cephalograms were traced and superimposed by the same investigator and analyzed by modified McNamara analysis and pitchfork analysis. Changes were evaluated with paired t-tests at a significance level of p<0.05.

Results: The maxilla and maxillary dentition moved forward. The mandible moved backward, although not significantly; and the mandibular dentition moved forward. The net dental changes combined with the apical base change resulted in a favorable total molar relationship correction. Net dental movement was 26% and the apical base change 74% of the total molar relationship correction.

Conclusion: These results suggest that TTBA has a maxillary protraction effect that can be useful in the treatment of growing skeletal Class III malocclusion with maxillary deficiency.
